The Caribbean Region tournament was first held in 2001, when the Little League World Series expanded from eight to sixteen participants. The Latin America Region -- which previously advanced one team to South Williamsport -- remained intact as an administrative region but was split into three regions for tournament competition. Mexico was granted direct access to the series, Central and South American countries remained in the Latin America Region, and island nations and territories throughout the Caribbean began competing in a new Caribbean Region tournament.
Little League's presence in the Caribbean began in 1958, when leagues were formed in Puerto Rico. Rear Admiral Daniel Gallery, Commander of the Tenth Naval District in San Juan, was instrumental in organizing the first chartered leagues on the island. Puerto Rico first participated in a region tournament, joining teams from Mexico and Venezuela in the inaugural Latin America Region tournament in 1958. The next year, Caparra Little League from San Juan became the first Caribbean-based league to advance to the Little League World Series.
After participating in the first few Latin America Region tournaments, Puerto Rican leagues were assigned to the Eastern Region tournament structure from 1963 through 1967. During this era, the island's champion participated alongside state champions from Northern New England states in an Atlantic Sectional tournament that fed into the New England divisional tournament.
Other Caribbean locales began forming leagues and entering tournament competition in the late 1960s and early 1970s. The U.S. Virgin Islands and Aruba began participating on an annual basis in 1969, the Dominican Republic in 1973, and Curacao made its first tournament appearance in 1975. Tournament participation from Caribbean leagues increased markedly after the Caribbean Region tournament was first held, with over a dozen different countries and territories sending teams to tournament competition.
The Little League World Series expanded further in 2022, growing from 16 to 20 teams. As part of this expansion, two additional Latin America teams gained direct series access, with two of Cuba, Panama, and Puerto Rico qualifying directly each year from 2022 through 2025 on a rotating basis. For 2026, Curacao and Panama's champions have direct qualification to the Little League World Series.
The Caribbean Region has won two Little League World Series championships, with both titles won by leagues from Curacao. Pabao Little League from Willemstad defeated Conejo Valley East Little League (Thousand Oaks, Southern California), 5-2, in the 2004 Little League World Series championship game, and Pariba Little League from Willemstad defeated Paseo Verde Little League (Henderson, Nevada), 11-2, to win the 2026 Little League World Series. In addition to these titles, Pabao won the international bracket championship in South Williamsport on four other occasions, finishing second in the Little League World Series in 2005, 2019, 2022, and 2023.
