In 1961, Oregon officials created Districts 5 and 6. Leagues that were previously in District 4 were placed in these two districts, and District 4 was re-formed in an area extending from the western portion of the Portland area down to the Salem area.
In 1976, leagues in the Salem area were split off into a new District 7, and the boundaries for District 4 were restructured to include only the northern portion of the district's previous boundaries. Today, the district encompasses points west of the Willamette River, including Tillamook and Washington Counties, Northwest Clackamas County, Northwest Yamhill County, and West Portland.
Teams listed in bold also won an Oregon state tournament.
